In 2008 the editor published a book related to this topic, titled Psychotherapy with Cardiac Patients: Behavioral Cardiology in Practice (American Psychological Association). Aside from that book, there are very few resources specifically written for clinicians who treat psychologically distressed cardiac patients. Unlike other medical specialty areas such as oncology, the field of cardiology has been slow to integrate behavioral treatments into the delivery of service. Perhaps because the field has been largely defined and dominated by researchers, mental health clinicians are only starting to recognize behavioral cardiology as a viable arena in which to practice. There is a large void in the practitioner literature on behavioral cardiology. In a review of Psychotherapy with Cardiac Patients, Paul Efthim, Ph.D. wrote, "Her new book goes well beyond previous works by giving specific and detailed guidance about how to tailor psychological interventions with this variegated population." He added, "It would benefit from even more details about treatment approaches." This proposed volume goes beyond the editor’s previous volume by providing in-depth descriptions of behavioral treatments for distressed cardiac patients written by eminent leaders in behavioral cardiology. This book describes a wide range of behavioral treatments for the common psychologically based problems encountered by clinicians who treat cardiac patients. The book is organized as follows: Part I focuses on the most psychologically challenging and common presentations of cardiac diagnosis; coronary artery disease, arrhythmia, and heart failure. This section also includes a chapter on heart transplantation, which is a treatment, not a diagnosis, but a treatment that incurs profound psychological impact for the individual. In Part II, behavioral interventions for the general cardiac population are described. Mainstream therapies such as stress management, cognitive-behavioral therapy, and medical family therapy are described, along with approaches that have less empirical support but considerable practical significance such as personality-guided therapy and interventions aimed at altering type D personality traits. The literature in behavioral cardiology has a rich history of investigating maladaptive personality traits and thus it is important to include behavioral approaches that target personality in this volume. Part III focuses on common behavioral problems encountered by clinicians who work with this patient population. Most patients who seek psychological help do so because they perceive themselves to be stressed due to their job or overextended in all areas of their life. Other people with heart disease present with sleep problems and/or an inability to motivate themselves to exercise or quit smoking. There are many practical behavioral approaches that can be helpful for patients with these difficulties and these are detailed in this section of the book. The conclusion of the book focuses on how to integrate the behavioral treatments described in the preceding chapters into a comprehensive treatment model.

About the Author

Ellen Dornelas, Ph.D. is associate professor in the Department of Medicine at the University of Connecticut School of Medicine at Farmington and director of behavioral health programs at the Henry Low Heart Center, Hartford Hospital. She received her Ph.D. from Yeshiva University in 1993. For the past 15 years, she has worked as a hospital-based psychologist, treating cardiac patients in medical inpatient, outpatient psychotherapy, and cardiac rehabilitation settings. She was trained as a clinical health psychologist and has a strong background in research in adapting psychotherapeutic approaches to meet the needs of medical patients. She edited a special issue of In Session: The Journal of Clinical Psychology called Integrating Health Psychology into Clinical Practice. In addition, for several years she was the media spokesperson for Connecticut’s anti-tobacco coalition.

Five Editions Says It All

The Trinity Alps are one of the most rugged, and least visited, mountain ranges in California. Despite their close proximity to the volcanic Cascades, the Alps more closely resemble a lower version of the Sierra, full of granite with some minor glaciation. They also have some geological similarities with the coastal range, and an almost unrivaled biological diversity, even considering their many rivals within the Golden State. But where to go to explore this range? You might consider starting with veteran Wilderness Press author Mike White's guide to the region and surrounding area. It is rare for a hiking guide to make it into multiple editions, but White's book is now in its 5th edition, and for good reason. It is a thoroughly researched and meticulously detailed guide to hiking in California's Northwest corner.

Checking in at well over 300 pages, this book not only covers the Trinity Alps Wilderness, but the equally scenic (in my opinion) Russian Wilderness, Whiskeytown Reservoir National Recreation Area, and the lands surrounding Castle Crags State Park. Most of the hikes in this book are extended backpack trips. White not only provides detailed route descriptions, but also the average elevation gained per mile on a hike, a statistic I find particularly useful. He also notes many side trips on each route, some of which involve overland hiking. But day hikers will also find a nice collection of options within the pages of this book. You can explore the many waterfalls around Whiskeytown Reservoir almost any time of year, including the famous lost falls. (A good feature of this book are the excursions into local history, including how a large waterfall came to be lost until 2004!) Finally, each route has a fairly detailed sketch map. That said, I would still use this book in conjunction with the US Forest Service Wilderness Maps just for the added detail a good topographical map includes.

On the whole, this is a fun (and educational) book to read, and should be your first resource when exploring the area on foot. It has been a staple of backcountry users for almost 30 years now, and there is a reason for that. It is simply the best guidebook to the region currently available.

Detailed but old fashioned

Just came back from the trinity alps where I used this book on a couple of routes. This is clearly a very professional book and I found it helpful if somewhat frustrating (see below).

The Table on Page xii is helpful to pick routes for their level of difficulty. If you look carefully you will see that the selection is pretty rugged. For our party I was looking for hikes between 4-8 miles in total length with (note!) less than 2,000 elevation change. There were very few (two) of those to choose from in this book.

I would like to make the following suggestions to the author:
* The map just before the table of contents is helpful. If there was a blow up version for each chapter with approximate locations of the different trailheads that would be very helpful for general orientation and base camp picking!
* In fact this being the 21st century one can complement every story of where a trailhead lies with a stable 20 character Google maps link to exact said location. This would make readers' lives so much easier when setting up one's GPS for the trip.
* Finally, I have only sampled two trails but in one of them (Tangled Blue Lake, #22) I was surprised at the lack of useful details in both story and provided map. For example, there were at least a couple fine dirt roads that split off from our route - neither was mentioned. At some point one has to leave a wide dirt road and switch to a narrow path. This switch is not discussed. etc.
* Because of the ragged landscape, it would be nice to denote where a shorter route on any of the suggested backpacking trips could lead to a pleasant day hike. (Ie, if the first 3 miles out of a 10 mile out backpack trip already leads to the first pretty lake).

Battling the Powers of Darkness

"Fire and Sword" is the fifth book in the "Chronicles of the Host" series. D. Brian Shafer continues in this exciting battle between good and evil. Lucifer and his angels strategize their next steps in their attempt to thwart the influence of Christ and His followers.

Shafer's descriptive word pictures brought a new realism to Paul's prison cell in Rome and insight into the parchment, quill, and ink process of his correspondence with the churches of the New Testament. During Luke's visits with Paul in prison, he fills Paul in on the recent events in Jerusalem regarding the amazing ministry Peter and the other apostles are experiencing.

The book backtracks to give an account of Paul's encounter with Steven and his stoning, and of his own conversion. Shafer follows with an account of the apostles' ministry as recorded in the book of Acts and of Paul's encounters while on his missionary journeys.

Conversations among the angles representing the powers of darkness clearly picture Lucifer's deceit and plans to dupe man by obscuring truth and with offers of worldly pleasure, and power. The archangels and the angelic hosts teach valuable lessons through their stimulating dialog.

Using creative, dramatic, and imaginative fiction as his platform, Shafer provides a brilliant balance of combining scriptural truth with literary license. His writing is Biblically sound, spiritually enlightening, and mentally invigorating.

"Fire and Sword" is a must read for anyone who enjoys historical Christian fiction, with a hint of fantasy and Science fiction. A great read.

I was very excited to get this book! It is part of a series of book from this author and they are awesome books. The best I've read on this subject outside of the Bible itself of coarse! The book came to me in excellent condition and I am very pleased. I have to say that Amazon has never let me down!

About the Author

DANIEL LENIHAN, winner of the George Melendez Wright Award for Excellence in Submerged Resources Research, has been diving as a park ranger and archeologist for the National Park Service since 1972, and founded the Submerged Cultural Resources Unit (SCRU) in 1976 before being appointed the first chief in 1980.

Over the last 25 years, Lenihan and the SCRU team have been the subject of national media stories and many TV documentaries on CBS, ABC, BBC, CNN, PBS, The Discovery and History Channels, and National Geographic. He has written frequently for Natural History and American History, and co-authored, with Gene Hackman, the well-received sea adventure novel Wake of the Perdido Star. A native New Yorker and former schoolteacher, he lives with his family in Santa Fe, New Mexico.

Enjoyable adventure

I think people of any age who enjoy adventure writing or history will like this book, which recounts the tales of a National Park ranger/diver. His job, along with his team, for more than twenty years, was to map underwater wrecks and preserve the sites for exploration by future divers. Along the way, he seems to have had a really good time. There is an interesting story in each chapter.
I am planning to give the book as a graduation gift to my nephew as I think he will enjoy, as I did, the stories about the joys and mortal perils of cave-diving in Florida, mapping wrecks in the Great Lakes in body-chilling 34-degree water, and close encounters with the slow moving - - but potentially deadly - - lion fish in Micronesia.
I also enjoyed chapters that show the author's awareness of the benefits and drawbacks of age in a young person's sport. I haven't gone diving in the English Channel - - 170 feet deep - - to explore a confederate wreck (the Alabama, which sank off Cherbourg, France, in 1864), but I could identify with the author when he realizes that his eyesight isn't, umm, quite as good as it used to be:
"As the dive progressed, however, I found myself coming face to face with my own aging process. At depth, I usually enjoyed the advantage that experience grants older divers. I could feel smug as I watched younger and stronger men make those myriad little judgment mistakes to which I am not as prone - having already made most of them myself during a quarter century of mucking about in deep water. Depth was, in a sense, the great equalizer. Then, without breaking our pace over the bottom, I reflexively reached for my gauge console and brought it to my face for a routine check of elapsed time and remaining air pressure. I couldn't read it."
By the way, the author copes with this difficulty on the next dive (magnifying glass inside the goggles).
Also, if you are browsing through the book, I recommend reading the chapter about diving at the site of the USS Arizona. The author, at first trying to keep his distance, gradually comes to terms with his feelings about the ship and the thousand or so young men who lost their lives on one bright day in Pearl Harbor.

Fascinating account of a career in passionate underwater conservation

Sometimes it's hard to tell by the title what a book is all about. "Submerged -- Adventures of America's Most Elite Underwater Archeology Team" certainly sounds interesting, but I wasn't quite sure about to the exact nature of the volume. Turns, out it is the recollection of the founder and former chief of the United States National Park Service Submerged Cultural Resources Unit, a group of National Park Service divers, scientists and other professionals seeking to document and catalog shipwrecks. The "SCRU team" is thus a legitimate part of the U.S. Department of the Interior, yet it is one that's about as far removed from stereotypical deskbound civil service as one can imagine. Over a period of 25 years, author Daniel Lenihan created and crafted a team of divers whose skills and sense of adventure was second to none, yet also a group that combined astonishing underwater feats with a keen sense of archeological and anthropological imperatives.

Lenihan describes his own introduction to cave diving as one of the pioneers who developed and advanced the state of the art when the sport was young and so many died in their often ill-conceived pursuits that the government considered closing off the Florida cave systems. Like most divers, young Lenihan was intrigued by finding and recovering artifacts but, unlike most, he quickly discovered that removing them meant destroying perhaps their most intrinsic value, that of learning from the past, the setting where they were found, the condition they and their surroundings were in. In the early 1970s he studied anthropology at the University of Florida, then joined the National Park Service as a "Park Ranger/Archeologist." Lenihan's quest essentially became a fight against the mindless destruction of shipwreck sites by treasure and artifact hunters by finding and documenting them so they could be properly protected as national cultural resources, just like those above ground.

The book, divided into three parts ("Caves, Dams, Shipwrecks, and Dreams;" "The SCRU Team;" and "Reaching Out") and 22 chapters, documents Lenihan's lifelong quest, their early missions, and how his team's influence and reputation grew until it was called to work in all parts of the world, often in conjunction with the US Navy and other governmental entities. We learn about the development of underwater surveying techniques, ranging from simple measuring and triangulation all the way to sophisticated high-tech scanning and mapping systems later on.

Lenihan describes such diverse operations as diving the frigid waters around Isle Royale (a national park in Lake Superior) to map and document the wealth of shipwrecks surrounding it; to doing the first actual underwater survey of the USS Arizona at Pearl Harbor; to locating wrecks around Fort Jefferson in the Dry Tortugas in the Gulf of Mexico; to potentially hazardous dives to the USS Saratoga at the bottom of Bikini Atoll that was used for nuclear tests in the 1940s and 50s; to discoveries around Micronesian islands. He describes almost impossible-to-get-to excursions into Kauhako Crater on Molokai; underwater searches in the Aleutians where tactical side-maneuvers had played a large role in the outcome of the more major seabattles of WW II; grisly rescue and recovery missions in poorly accessible locations where even Navy divers deferred; and making sure French divers properly surveyed and protected a sunken Confederate raider, the CSS Alabama, in the English Channel off the coast of France. Learning, developing, training, passing on always figure large in Lenihan's work, as does a healthy respect of the dangers of diving, and the ensuing meticulous preparation and following of diving protocol and procedures. There are many other examples, all wonderfully described in Lenihan's style that merges good storytelling with precise technical information and always a nod of appreciation towards those who helped him and his team, plus a good deal of pride in their accomplishments.

"Submerged" presents all of this in a holistic way -- recollections, experiences, reports, suggestions. Lenihan includes adventures of his youth, including cave diving trips to Mexico with such pioneers as Sheck Exley who later perished in one of the very caves they had explored, as well as hopes for the future.

This is a book about diving both as a passion and as a tool for the greater good of mankind, in this instance the preservation of underwater heritage. "My conviction, which has emerged from thirty years of diving, is that shipwrecks and underwater caves are places where one can touch the past in the most special ways," writes Lenihan who also described himself as someone who once "associated with professors and students who thought SDS, SNCC, and Abbie Hoffman were too damn conservative." Out of that counter-cultural mindset grew a sense of responsibility for our submerged heritage, and the drive to make it real, that sets a shining example of what can be accomplished when passion and purpose merge in a career, and that fortunate synthesis Lenihan successfully shares in this eminently readable and highly recommendable book.

Risky business!

I am an experienced technical diver and was fascinated with that aspect of this book. Mr. Lenihan is indeed a good story teller. I wouldn't be caught dead doing some of the dives that they did on air-- but then again they were diving years ago when no mixed gasses were easily available. I feel that I have the right to take souvenirs from shipwrecks if I've gone to the trouble and expense to get to them and they're going to just corrode away in the sea. But Mr. Lenihan makes his points about preservation without being obnoxious and self-righteous and I like that. He made me think enough about the value of these wrecks that even though I'll probably still take small souvenirs, my newly informed conscience would keep me from taking anything too nice. Don't buy this book if you want to know the best and safest ways to deep dive or cave dive. I'm not saying they aren't real good divers but they dive with air and a prayer. Still, in all, I really enjoyed it.

About the Author

Award winning journalist and author PALLAVI AIYAR spent six years living in a hutong home in the heart of the old imperial city of Beijing. She reported from across China for the Hindu and Indian Express in addition to teaching English at the Beijing Broadcasting Institute. She is the winner of the 2007 Prem Bhatia Memorial Award for excellence in political reporting and analysis for her dispatches from China. Her book Smoke and Mirrors: An Experience of China won the Vodafone-Crossword Popular Book Award for 2008. Her first novel, Chinese Whiskers, has recently been published by Harper Collins India to excellent reviews. She currently lives in Brussels with her husband, son and two Chinese cats, where she writes about Europe for the Business Standard. Pallavi has degrees in Philosophy, History and Media Sociology from St StephensCollege Delhi University, Oxford and the London School of Economics.

Charming story and perceptive insight into modern China

This is a charming story for children and adults alike. I loved it, my 10 year-old loved it, and I read it to my 6 year-old, who loved it too. They even learned a bit of Chinese! It's a great way to introduce children to contemporary China. The story is told from the perspective of two cats, so it's appealing for children, but it's no fairy tale. It's a perceptive insight into the social conflict created by increasing inequality, and the damaging effects of runaway growth and corruption. Aiyar brings backstreet Beijing to life with a convincing cast of characters coming to terms with the pace of change. One for all the family.

a bit far-fetched

I wasn't sure what to expect going into this novel other that knowing it was narrated by (2) cats who live in Beijing. I was thought might be a pleasant change from all the stories I've read that were told from a dog's perspective.

The (2) cats are former strays who have lead very different lives prior to be adopted by a foreign couple who named them Soyabean and Tofu. Soyabean is a ginger-colored, handsome male -- picture future model for a pet food company. He grew up as a stray in a middle class area. Life on the street was easy for him compared to that of Tofu. Tofu, is a petite dark colored female. She was the only female in a litter of five and sadly had been a dust bin kitten. A kind old man fed her mama scraps of food and an occasional bowl of milk , but life on the streets was rough. Tofu's mom told her and her brothers that they were descended from royalty and that she had a wealthy life-style until the communist revolution changed things.

Their new owners were kind to the two cats and fed them well, but suddenly their world changes when the SARS outbreak hits, and, the tainted pet food scandal makes it unsafe for cats to roam outdoors. Hysteria runs rampant and stray cats everywhere are being rounded up.

In a nut shell, this book was just okay IMO. Told in alternating chapters from each cat's POV, I initially enjoyed the insight into the Chinese culture and beliefs. Some of the cats musings left me with something to thing about, however brief. Added to the book was a short glossary of Chinese words and their meanings which was a plus.The story seemed a bit strange overall, but in the end the cat's triumph.

Pallavi Aiyar's first book on China, "Smoke and Mirrors", provided a fascinating insight from the perspective of a young Indian female correspondent discovering and living in Beijing and travelling through much of the country. It brilliantly combinet autobiography, anecdote and analysis. In "Chinese Whiskers" Pallavi has written a remarkable allegory, a narative looking deep into the soul, heart and body of China, through the eyes (and whiskers) of two remarkably intelligent, intimate, articulate and affectionate cats. It is an admirable feat. And without doubt deserves the encomium of being one of the best recent books on China; certainly there are not many like it, it is its own unique genre! About the Author

Matt Bell's first story collection, How They Were Found, was published in 2010 by Keyhole Press, and was reviewed in The Believer, American Book Review, and Bookslut, among many other venues. In 2012, Cataclysm Baby, a novella, was published by Mud Luscious Press, with blurbs by Karen Russell, Lucy Corin, Lance Olsen, and Chris Bachelder. Bell's fiction has been anthologized in Best American Mystery Stories 2010, Best American Fantasy 2, and 30 Under 30: An Anthology of Innovative Fiction by Younger Writers, and shortlisted in Best American Short Stories 2010 and the Pushcart Prize anthology. He serves as Senior Editor at Dzanc Books and teaches writing at the University of Michigan in Ann Arbor.

A Stark Fable Set in the Dirt

Matt Bell's novel takes place in a mytho-poetic realm that appears to be an extended metaphor about marriage. In an isolated house in some dirt by a lake -- all of which have their own secrets -- a man and a woman try to start a family. Bell eschews specifics to create a kind of Green World, a forested area of dreams and nightmares in which the man's hunting and fishing, the woman's ability to sing objects, stars and even a moon into life, and a mysterious and threatening bear all play a part. The couple try to have children, but all their attempts save the last end in miscarriages -- the first, through the bizarre act of the husband, becomes another character, a kind of anti-conscience prodding the husband to lose his love for his wife.

The novel is at times hypnotic, at times repetitive and at times gross. With characters this mythic, one comes to believe that Bell is saying something about marriage and children, and readers will come to their own conclusions about what that statement is. In the meantime, you'll need to adjust your reading to Bell's unusual style.

While the praise this novel has garnered is somewhat overwrought, even by the hyperbolic standards of blurbs (come on: Calvino? Borges? Kafka?!), the book is very, very good, and at least one blurber, Benjamin Percy, gets it precisely right: "Matt Bell does not write sentences--he writes spells. . . . This book . . . will grip you in an otherworldly trance." And that's just what Bell's prose does. It grips you. Its weird constructions and odd juxtapositions, its jarring cadences and surprising rhythms entrance you, weave a spell over you, almost from the very first word, and the book doesn't let go until the very end, until Bell has told his strange, inventive tale in his strange inventive prose. No: it doesn't let go even then.

A Fever Dream About Fatherhood

Reading like a twisted fairy tale, Matt Bell's debut novel is about a newly-wed couple who leave the bustling city that had been their home to make a new life in the land on the other side of the mountains. Embracing a life of simplicity, the husband, our narrator, builds a house upon the dirt using only basic tools. His wife furnishes their humble home with the objects she magically sings into being. The young couple desires to start a family, but the wife's repeated miscarriages lead to deep splinters in their relationship.

This is a tricky book to write about. I'm not sure how much I should say about the plot, so I'm not giving away anything that isn't already in the book's blurb. This is really only a tiny portion of the novel, but if I went on any farther, you would probably think I was talking about a strange fever dream and not an actual book. It's that surreal. I will say, though, that there is a giant bear in the woods, a squid/whale in the depths of the lake, and a labyrinthine series of chambers filled with memories under the house.

In addition to being unsure of how to write about this book, I'm not entirely sure how I felt about it. On one hand, the writing is absolutely beautiful, and man can Bell construct a sentence. His words are poetic and the book's tone evokes the magical fables of old. On the other hand, this book is very dark and disturbing; there were some grotesque descriptions that made me feel nauseated. Let's just say they involved dead fetuses, ghost children, and horrible, disfiguring injuries. This is not a book for the faint of heart.

I also thought there were some problems with the plotting and pacing. I had no idea where this book was going most of the time, and I would have liked more momentum to propel me forward in my reading. Come to think of it, though, this actually fits pretty well with the surreal, dream-like quality of this narrative.

In the House Upon the Dirt contains really interesting ideas about desire, regret, marriage, and fatherhood, but sometimes they felt muddled and repetitive. However, Matt Bell does have a very strong voice, and there is a burning power behind his words.

I usually love books with elements of surrealism or magical realism, like The Tiger's Wife by Tea Obreht, No One is Here Except All of Us by Ramona Ausubel, Understories by Tim Horvath, anything by Karen Russell, and some of Murakami's books, but I think this one was a bit too warped for my taste. I would recommend this novel to fans of fabulism who also have a strong stomach and an interest in the dark side of the psyche.
